For those of you that would like to buy spirits wear matching your little player's jersey below is the order form. Your will need to turns this in to the concession stand no later than May 11th... 

Instructions : 

1. choose your color 
2. write your name and your phone ( preferably your cell phone)
3. T-shirt size. here you want to request the number of t-shirt per zie ( ex.: 3 youth - m )
4. Name and/or numbers addon. here you will specify which shirt you would like to add the name of your player and his or her number. as per the form name is $3, 1 number is $3 and a 2 digit number is $6
