Divisions Include
Freshman - 1st/2nd Grade
Sophomore - 3rd/4th Grade
Juniors - 5th/6th Grade
Seniors - 7th/8th Grade
Must be entering selected grade for the 2016/2017 school year.

Registration Documents and Forms
Our program is part of the Northwest Junior Perrier Football Association, which operates under the rules of American Youth Football (AYF). These rules have been developed over time to ensure a fair and level playing field for all participants. The paperwork requirements below must be satisfied before a participant will be issued equipment or allowed onto a practice or playing field.
General Requirements
- Copies must be legible
- All AYF forms are available on the Home Page under Important Documents
2016 Registration Required Documents and Forms
- Copy of birth certificate
- Must show an original birth certificate to the registrar or designee AND submit a copy.
- The participant’s name and date of birth must be clearly visible and must match registration information.
- The raised stamp or seal should be visible on the photocopy.
- Copy of school report card
- Participant’s name, the name of the school and the school year must be clearly visible.
- This must be the YEAR END report card, ending in the spring of 2016
- Copy of Physical Exam or AYF Medical Clearance
- (Only required if pertisipating in AYF Turniment or Event.)
- Participants name and date of exam must be clearly visible
- If the date of the physical exam is between January 1 and July 1, 2016, a copy of the physical exam alone is acceptable.
- If the date of the physical exam is prior to January 1, 2016 or after July 1, 2016, the AYF 2016 Medical Clearance form must be signed, stamped and dated by the child’s healthcare provider.
- AYF Emergency Medical Treatment Consent
- The AYF 2016 Emergency Medical Consent form must be completed and signed by parent or guardian to provide a quick reference for medical personnel in the event a parent or guardian is not present or available.
- All information will be kept confidential

- Parents and players must sign, initial, and return only page 2 of the AYF 2016 Participant Tracking form.
- Parents do not need to attach a picture. We will upload pictures after players are assigned their jersey number for the season.
