DISCIPLINARY & TARDINESS POLICIES & PROCEDURES AGREEMENT

 Disciplinary Procedures

 If your child is disruptive, behaves in a reckless or unsafe manner, not willing to follow directives from supervisors, not willing to follow program rules, uses vulgar language, is disrespectful toward other participants and/or staff, parents will be notified of the incident when you arrive to pick up your child.

 ***Please note: If any of the above listed behavior is extreme, violent, or could cause an unsafe environment to themselves, supervisors, or other participants, the parent will be called to collect their child immediately.  Child will not be allowed to return to the program for the remainder of that week.  NO REFUNDS AND/OR CREDITS WILL BE ISSUED for a partial week.

 A second incident of any of the above behaviors will result in a written warning and parent/guardian/emergency contact will be called to collect your child immediately.  Child will not be allowed to return to the program for the remainder of that week.  NO REFUNDS OR CREDITS WILL BE ISSUED.

 Further unacceptable behavior (either to themselves or other participants) will result in the removal of your child from the program.

Tardiness

Please be prompt when dropping off or picking up your child/children.  Please be considerate of the staff.  It is extremely important that you pick up your child on time.

 We will not leave a child unsupervised; your emergency phone number(s) will be called if you are more than ten minutes late.  If we are unable to reach any of the contacts you provided and/or they live “out of state” or more than a 10- minute ride from the program location, and/or your contact is unable to pick up your child, the BELCHERTOWN POLICE WILL BE CALLED.

 Repeat offenses by parent/guardian will result in the removal of your child from this program.  No refunds or credits will be issued for a partial week.

 I, _______________________________________ , have read and understand the above rules and policies concerning the conduct of my child during this program.  I have also read and understand my responsibility in retrieving my child promptly.  I have read and understand that my child may be removed from this program for the above stated reasons.  I have read and understand that refunds and/or credits will not be issued.
