Dear Mr. Rodrigues,

Florida Youth Soccer Association (FYSA) presented your request for a name change to the Board of Directors at its most recent meeting.

FYSA takes great pleasure in informing you that the outcome of the vote regarding your name change was a positive one. I would like to commend you for submitting all documentation showing that you have already obtained the necessary licenses in Florida so that Central Pasco United Soccer Association may do business as Greater Tampa Bay F.C.

You have complied with all requests that would be made of an affiliate requesting a name change.
