Spring 2016 signups

Where: Sissonville High School cafeteria.
When: Every Saturday in February 9am to noon....
Cost: $70 first child, $60 for second, and $50 for third.
Late Fee: $25 registration ends Feb 27th (t-ball ends 2 weeks later)

Required Forms: Proof of age (Birth Certificate)
3 proof of residency- (I will include a link listing approved documents) OR School Enrollment form filled out by the principle, vice principle, or administrator of your child's school (has to attend SES, Flinn, or SMS)- link to that form included.
