Registration Requirements

  1. Registration Form completed and signed by parent/s.   
  2. Emergency Medical Authorization completed.   
  3. Acknowledgement of Risk form must be read, understood and signed.   
  4. An athlete MUST comply with all rules and regulations that District 3 Little League and Northwest Little League has set forth.   
  5. Photo copy of birth certificate.   
  6. All students have to provide a current copy of progress reports along with report cards. Failing grades mean you fail the team. If he/she is on academic probation (meaning failing 1 or more classes), the athlete will have to work extra hard to pull grades up during the Spring and late Summer baseball season.   
  7. ALL FEES must be paid in full prior to the first scheduled contest for each season. It shall be understood that payment of fees does not guarantee athletic participation for every inning of every game. If an athlete drops or is removed from a program, such fees paid shall not be reimbursed.
