For participation, all players are required to provide the following completed forms and a copy of their birth certificate. If you are a returning player, we should have a birth certificate on file and there is no need to supply a new one. Birth certificates can be uploaded in the player registration database.
Please print and complete forms and bring them to commitment night for your team. If you are unable to attend commitment night, please return to the Villarreal Force Academy office as soon as possible. Forms can be returned in one of the following ways:
SCANNING AND EMAILING: Please scan each document separately and name them according to the player's name. Preferred file type is .pdf or .jpg. Send as attachments to
[email protected]DROP OFF OR MAIL: It can be dropped off or mailed to 445 Treetop Drive, Fayetteville NC 28311. If the office is not open at the time you drop by, please leave in the black mailbox outside the door by the soda machine.
