How to Register your team

League Fees pay for team insurance, PONY dues, 2 umpires per game, game baseballs, practice fields*, game field rental, lighting, maintenance, prep, and equipment***

Mandatory Coaches Meeting - Tuesday, August 6th at 6:00 PM 
Must have at least one team representative in attendance, $300 deposit, along with Team Registration Practice Request due at meeting
NO PRACTICE FIELDS WILL BE ASSIGNED UNTIL FULL LEAGUE DUES ARE PAID.  Game balls given to those in attendance.

Under the Forms Tab complete the following forms and EMAIL TO [email protected] & [email protected] AS PDF ATTACHMENTS:
Due at registration:
1) Team Registration/Practice Request* form (one per team) DUE AUG 6!
2) Mangers Application (one for each coach/manager) Returning AZPlayball coaches only complete for new coaches listed on your roster
3) Concussion Training certificate, if not previously submitted in 2018, course link below** 
4) $300 Team Registration deposit DUE JULY 31 See Payment Options below 

Due at first practice:
 

5) Player Roster completely filled out (emailed as an Excel spreadsheet attachment Subject: Roster for "Your Team Name")
6)) Medical Releases (keep your originals to have at every practice and game)
Send the copies separately from your Rosters, Subject: Medical Releases for "Your Team Name"
7) Balance of League Dues due by AUG 12!

2) Send by PayPal to [email protected] FRIENDS AND FAMILY OPTION ONLY (money sent Goods and Services will incur a 2.3% fee)
3) Credit Card payments will incur a 2.3% fee.
4) The balance of League Dues must be paid to have a practice field assigned. 


**Concussion Training - All managers and coaches must complete the NFHS on-line concussion course (no cost) and forward a copy of the certificate to Playball prior to the teams first practice. A link to this is at the top right of our home page.
