BLL Medical Release Form

BLL Baseball & Softball Medical Release Form Submission Instructions:

All players in all divisions of Berlin Little League must complete a the Little League Medical Release Form for the Spring 2024 season. Completed health forms must be printed out in hard copy and submitted to your player's head coach at the first practice of the season. Reminders will be sent out from your team coaches once team assignments have been announced.
