Below are the following forms that must be submitted upon registration:

2023 Participant Contract and Parental Consent
         ****Please use given name as it reads on birth certificate

2023 Physical Fitness & Medical History Form
 ****Physical form must be filled out by a doctor and either signed or stamped
 ****Blood pressure must be listed (i.e. 120/80) and not just checked

2022-2023 Official Report Card
 ****Must have all 4 quarters from the 2022-2023 school year, no exceptions

Official Copy of Original Birth Certificate for Participant
****Original birth certificate and copy must be present at player verification 
****Original will be returned after player verification

All coaches and volunteers are required to complete a background check yearly. This is for player safety.
