Forms must be digitally submitted through the registration portal, all forms can be filled out electronically  with the exception of the physical.  The Physical will need to be printed and signed by the physician, then uploaded. 

Note that in order to digitally sign each from the forms will needed to be opened using adobe acrobat, otherwise the signature line may not be enabled.
