The OHS All Sports Boosters are proud to provide 3 annual scholarships to students that have parents who are current members of the OHS All Sports Boosters.

OHS ALL SPORTS BOOSTERS SCHOLARSHIP AWARD APPLICATION

CONDITIONS FOR APPLICANT

1. One-time non-renewal award
2. Graduating high school senior
3. Enrolling in a university, college, or accredited school in order to pursue further training or education
4. Must have participated in at least one interscholastic sport
5. Award is payable upon certification of college or trade school

INSTRUCTIONS TO APPLICANT

1. Application must be completed in full
2. Grade transcript (available in Career Center) attached
3. Record of sports participation (use back of this form)
4. One to two page hand written report on “What being a member of the OHS sports team has done for you.” Attach to this application.
