I understand that it is a privilege to participate in all extracurricular and athletic activities at Christ the King

Catholic School. Accordingly, I agree to accept and to live by the following extracurricular activity codes:

1. I will work hard, during the season of the extracurricular activity or sport to improve my skills and be an

asset to my club or team.

2. I understand that my participation is a season-long commitment to myself, to my club or team, and to my

club teacher, leader or coach. If I quit the club or team at any time after one week following the

posting/selection of club or team members, I will not be eligible for any other club or sport at Christ the King

for the rest of the current school year. Should I decide to quit within the first week after the posting/selection

of club or team members or if quitting is due to unusual circumstances or an emergency, I will meet with the

club teacher, leader (for a school club or academic team) or with the Athletic Director (for a sports related

team).

3. I will show proper respect to my fellow club or team members, to my club teacher leader or coach, to any

parent assistants or chaperones, and to any judges, officials, or referees during all competitions. I will conduct

myself in such a manner as not to discredit myself, my club or team, and my school while at Christ the King or

when visiting other schools. I understand that my behavior is a reflection upon myself, my club or team, my

club teacher leader or coach, and my entire school.

4. I will try, as much as is possible, to be at every meeting, practice, and competition during the season. I

understand that if I miss meetings or practices that are unexcused three times during the season, I may be

removed from the club or team. If for some important and valid reason, I am not able to attend a meeting,

practice, game, or competition, I will contact my club teacher, leader or coach no later than the morning

before the meeting, practice, game, or competition.

5. I will be at all meetings, practices, games, and competitions on time.

6. I understand that if I am placed on “Academic Probation” (for maintaining lower than 70% in any academic

subject at the time of formal grading periods which are mid- and end-trimester), I will become ineligible to

participate in all clubs or sports for the next two weeks. At the end of two weeks I will meet with the Principal to

evaluate my academic progress and my eligibility may be restored if I no longer have grades below 70%. If I

still have grade(s) lower than 70%, my grades will be checked every two weeks to determine eligibility.

7. I understand that as a student club member, team member, or athlete, I must maintain a good disciplinary

record. If I am referred to the Principal for behavior reasons, I may be placed on “Disciplinary Probation” and

become ineligible to participate in clubs or sports for two weeks. At the end of the ineligibility period, my

disciplinary progress will be reviewed and my eligibility may be restored if I am no longer on DisciplinaryProbation.

8. I will organize my time so my academic responsibilities do not conflict with meetings, practices, games, or

competitions. I understand that my club or team commitment does not excuse me from maintaining

academic excellence and that my presence in school is required for my success.

9. I understand that if I am unable to participate in my regularly scheduled physical education class during

the school day, I will be ineligible to participate in after school sports related practices or games. If I go home

early from school due to illness, medical reasons, or for any unexcused reason, I will also be ineligible to

participate in after school meetings, practices, games, or competitions.

10. I understand that if I am excused from school early to attend a club or team competition or an athletic

contest or game, I am still responsible for all class work and any homework assignments given that day. I also

understand that I must be proactive and check with my teachers in advance so I know what I will be missing.

11. I will be personally responsible for all school equipment and/or uniforms issued to me and will return these

items in good and clean condition on the scheduled return date. I understand that if I don’t do this, I will have

to pay for the replacement of these items.

12. I understand that if I am a member of an outside club or sporting team other than Christ the King, that

Christ the King games, matches, and competitions must take priority. I will have to work with my outside club

or sporting team leader or coach to make arrangements related to any conflicts.

13. I understand that the club teacher, leader or coach has the sole responsibility to decide the make-up of

the group or team including who will participate/play in a game, match, competition, or contest.

14. I understand that any unsportsmanlike conduct or other violation of this agreement may result in

disciplinary action, not only by the club teacher leader or coach, but also by the school Principal or Athletic

Director, in the form of possible suspension or dismissal from the club or sport. I also understand that if I am

ejected or removed from a game, match, competition, or contest by a judge, referee, or an official, I will not

participate in the next scheduled game, match, competition, or contest (league, non-league, or playoff

game), and will be subject to any other disciplinary action deemed appropriate.

This Extracurricular/Athletic Agreement is to be signed by each student participating in a club or team during

the 2025-2026 school year and by their parents. A copy of the signed agreement will be provided to the

student and the original will be kept on file. By signing below, I agree to abide by all of the specific items listed

in this agreement.
