Team Documents and Check-In

Saturday Morning Check-in:
Prior to your first game on Saturday, please bring 1 copy of your team roster and any guest player roster approvals, along with all player cards to the nearest field marshal tent for check-in.  Your player cards will be zip-tied together to indicate they have been checked and are valid for the tournament. Please make sure your cards are laminated and hole-punched.

 

Pre-Tournament Registration

Coaches and Managers,

All team document uploads will be done online through the gotsoccer.com page.  All documents must be uploaded by September 17, 2021 (September 24 for Boys). Please follow the directions listed below.

Team Check-In:
1.     Log into your gotsoccer.com team account.
2.     Click on registered tournaments (Fall Ball Classic).
3.     Click "documents" tab (far right of screen).
4.     Upload:
a.  Official state approved roster (Please cross off any players not attending the tournament.  If you have a pool team, please cross off
all players not playing for the specific team before uploading).
b.  Approved Player & Coach cards (Front & back if player/team information is on both sides). Please scan multiple cards per page.
c.  Permission to travel (if your team is carded outside of Ohio Soccer Association. Not required if roster is US Club Soccer). Teams from outside Ohio are required to provide an approved travel permit from their respective state association.  This includes both Kentucky carded and Indiana carded teams participating in Cincinnati area league play. Please check with your respective state association for the process required for your state.
d.  Guest Player Approval Roster (from your state association). The maximum number of guest players for U8-U12 teams is (4), U13-U15 teams is (5).
Addition of Guest players may not exceed max roster counts:
U8-10: 12.
U11-12: 16.
U13-U14: 18
U15:  rosters may have up to 22 players, however if any guest players are included, roster may not exceed 18
e.  Guest Player Cards (Front & back if player/team information is on both sides).
f. Fall Ball Liability Release & Compliance Form (download below). Only one (1) form per team will be accepted.  Please list
players in order as they appear on the roster. Guest players must be included.

Additional requirements for all teams outside of Ohio:

-  Parent Concussion Acknowledgement (Ohio Law - Send all parents the Ohio Dept. Of Health Concussion Information Sheet, download
below).

g.  One copy of the coaches concussion training certificate for each coach (Ohio Law).  If you do not already have a valid concussion certificate, Please visit https://www.nfhslearn.com/courses/concussion-in-sports-2  Each coach will need to complete this course and then upload the certificate.

Medical releases for players do not need to be uploaded, however we request that you have them on-hand at the tournament.

Teams should bring all player cards and 1 copy of your roster (cross off any players not attending) and guest roster to the field marshal prior to your first game on Tournament day. The Field Marshals will check and zip-tie your player cards to be given to the referees at each game of the tournament.
