Residency Documents


TEAM  REQUIREMENTS

Each team is required to submit the following documents for ALL tournaments.  
These documents are required to be verified and signed off by the League President 
and Division Player Agent prior to submission to the District.  
Uptown Long Beach has requested that all tournament documents are to be turned in and verified by 
President Andy Hernandez and Division Player Agents by TBD.

- Player Birth Certificates (Original)
- Player Residency Documents or School Enrollment Proof
- Player Medical Release Forms
- Manager, Coach, and Team Parent Safety Cards

Baseball Team Affidavit

NOTE - This is a 2016 Form, there is not a 2018 Form published yet.

* The Team Affidavit is required to verified by Uptown Long Beach Little League President and Division Player
                        Agent.
   Without a completed Team Affidavit, the team will not be eligible to play in a tournament.
** Three (3) copies of the Team Affidavit will be required to be signed ***

 

* Information that will be needed for Team Affidavit

Player Age Charts
 You may utilize the Little League International link to determine playing age or the Player Age Chart.

Special Game Supplements
 
For 50-70 and Juniors, if a player was added to the team after the High School or Middle School
baseball season in which they were a member of that team, then this form must be completed to
show games played and where with the High School or Middle School team to qualify for tournament
play.

Coach Documents

Safety Card or Proof of Safety Class
 Include copies of cards showing the class was taken within 3 years in your Tournament Book.

Concussion Protocol Class
  Include copies of certificate showing the class was taken in your Tournament Book.

                *** Remember this class is required by law for all coaches and managers.  It is an easy 30
     minute on-line class to take.

* Player Proof of Residency is required for all players on the team and must be verified by the Uptown Long Beach Little League President on the Tournament Player Verification Form.

* All associated documents with a residential address within the league boundaries (minimum of 3, but only
   one from each category, dated between February 1, 2017 and February 1, 2018) are required to be attached
   to the Tournament Player Verification Form such as:

* To assist the manager in completing the Team Affidavit and ensuring that the documents collected from the players are correct, you may utilize the Tournament Verification Form Checklist.

 School Enrollment Form (Download School Enrollment Form)
* In order to qualify for a school waiver, the player/student must complete a 
  School Enrollment Form and have it signed by the school principal or assistant principal.

Boundary Waiver (Download Boundary Waiver / Moved Form - Regulation II)

* The Boundary Waiver Form is required if the player resides outside the league boundaries,
   BUT lived within the league boundaries prior to the season start date.

  
*** Please take note, that during ALL STARS or SELECT Tournaments,
ONLY THOSE PLAYERS THAT RESIDE WITHIN THE UPTOWN LB BOUNDARIES WILL BE ELIGIBLE.

 


          PLAYER MEDICAL RELEASE FORMS     (Download Medical Release Form)        

Player safety is of the utmost important.  But on those occasions where a player may become injured,  the proper
treatment and treatment facility must be given to the player.  Therefore, Uptown Long Beach Little League utilizes the
following form that is carried by all teams to authorize the medical treatment of any player that is injured in the event that
the parent or guardian cannot be reached.

Please ensure that the team has a current Medical Release on file.



Map Builder

RULES


Note that some of the rules for tournaments are different than the rules most leagues play with
during the season.

It is your responsibility, as a manager and/or coach to know the different and additional rules.

Please read the Tournament Rules portion of your Little League Baseball book, located in the back.

Pitch Count
Baseball divisions are required to monitor and keep track of pitch counts and rest days for each pitcher. Maximum allowable pitches (per game) are based upon age. Rest days are based upon pitches thrown in a game.
Managers and coaches are required to know and understand the pitching rules.
Pitch counts are required to be documented on The Pitch Count Form, and are to be available for inspection by the opposing manager, umpire, or league official at anytime.
The Pitch Count Form will be required to be turned in and verified by the league official at the beginning of each tournament game, along with a team line-up.

Little League Basics for Pitch Count Maximum Pitches Pitches Thrown Required Rest Days
Age 7 - 8 = 50 1 - 20 0 Age 9 - 10 = 75 21 - 35 1 Age 11 - 12 = 85 36 - 50 2 Age 13 - 14 = 95 51 - 65 3 66 + 4
Pitchers CANNOT play catcher if they threw 41 or more pitches in a game. Catchers CANNOT pitch if they played catcher 4 innings or more. One pitch in an inning equals an inning played.
If a pitcher reaches their maximum pitch count, or a threshold, during a batter, they may finish that batter and will only be required to rest the number of days that coincides with the threshold reached. EXAMPLE - Pitcher reaches 35 pitches during a batter, but only has a 2-2 count. Pitcher throws 3 more pitches to end that batter, so he has a total of 38 pitches thrown. He is only required to rest to the 35 threshold limit of 1 day.
