*Coaches/Team Managers must have a copy of every player’s birth certificate at hand should a league official request it.
MEDICAL & LIABILITY WAIVER: BY SIGNING ABOVE my heirs and I hereby release NEMS SOCCER LEAGUE and its employees from any liability due to illness, loss of personal property, injury, and any other legal entanglement during the above soccer Tournament. In the event of any emergency I authorize NEMS staff to seek immediate medical treatment for my child in case of an accident or illness and understand that any medical expenses incurred and my responsibility.
MEDIA RELEASE: BY SIGNING ABOVE OR CLICKING YES IN THE TERM REGISTRATION FORM I understand that images may be taken of the players in action on the soccer field. I give permission for my child to be videotaped or photographed for purposes of promoting NEMS and the game of soccer. We will notify the parents in advance if there will be any publicity opportunities.
