Important Registration Documents to bring to Document Submission Dates

- Two (2) Completed Player Contracts with photo attached (no stapling/taping)
  (Headshot must be unobstructed i.e., no hats, sunglasses, etc.) 

- 1 Original with COLOR photo (features unobstructed)
1 copy (visible black and white is okay for copy)

- One (1) Birth Certificate Copy (NEW participants)

- One (1) Physician Completed Sports Physical – original required
*Chiropractor or Holistic/Homeopathic/Naturopathic conducted sports physicals are NOT acceptable

- One (1) Copy of “End of school-year” report card (retained by SDYFC for Little Scholars)
*Progress reports not accepted

- Proof of Residency; Gas, Water, Electric, or Landline Phone Bill only (NEW participants)
*Any other document submitted that is not listed will not be accepted
