- Completed SDYFC 2022 Player Contracts

- One (1) Birth Certificate Copy (NEW participants only )
*Please also bring original for verification 

- One (1) Physician Completed Sports Physical – original required with stamp OR business card.  
        *Please note, Chiropractor or Holistic/Homeopathic/Naturopathic conducted sports physicals are NOT acceptable

- One (2) Copy of “End of current school-year” report card (retained by SDYFC for Little Scholars)
        *Will collect in June
*Progress reports will not be accepted

- Proof of Residency; Gas, Water, Electric, or Landline Phone Bill (NEW participants only) 
*Any other document submitted that is not listed will not be accepted

- Waivers
*Will be emailed once registration is paid in full.
