Derby Little League Merchandise Fundraiser

Fill out attached form below with name, contact information, and type of payment.
Enter a quantity for any items you wish to purchase and circle a size.
Enter your total order cost at the bottom and submit to any league official.
We will be available to collect forms at our clinics in February 2/12, 2/18, & 2/26 and at tryouts on 3/3 & 3/4 all at the DHS gym.
Completed forms and payments must be received by 3/4.
League order will be placed on 3/5 with the expectation that items will be received by Opening Day.
