PARENT CODE OF CONDUCT
Copy of BIRTH CERTIFICATE
Copy of 2024/2025 SCHOOL REPORT CARD (must show all 4 quarters)
If enrolled in Home School complete the 2025 Home School Form
All forms and payment must be submitted by July 31, 2025 in order for your child to participate on the first day of practice August 1, 2025.
