Cheerleading Required Paperwork

1. Original Birth Certificates (with seal for new cheerleaders)
2. Physical Dated 01/01/2025 or later
3. Medical Clearance (if physical is dated before 01/01/2025)
4. Final Report Card (2024-2025 school year)
