2023 Medical Physical & Emergency Authorization Forms - These two documents must be turned in prior to begining organized practices - no exceptions. The physical form must be completed and signed by a physician. Cheerleaders are not required to complete a physical. 

Lindsey's Law Forms (Sudden Cardiac Arrest) - Required for all players. 

SWBL Family Sportmanship Agreement - Every family with a player / cheerleader in peewees must complete this form.
