•  Player Contract/Parent Consent - Must be completed by a parent & uploaded during registration.
• Medical History Form - Must be completed by a parent & uploaded during registration.
  
• Physical Form - Pop Warner Physical form must be completed by Physican and uploaded by August 1st.
• Copy of Birth Certificate, State ID or US Passport - Must be uploaded during registration.
• Copy of Final 2024-2025 Report Card - Uploaded by August 1st

All athletes, both football and cheerleading, must submit these required documents before participating in any practices. By August 1st, all documents must be uploaded to your SportsConnect account/application, and copies must also be provided to the team coaches for football and the Cheer Director for Cheer after uploading.
