1. 3 forms of address validation in digital form

Examples: driver's license, utility bill, water bill, screenshot of address and student name in PowerSchool, mortgage bill, passport
***we know this is annoying but this is what the national little league is requiring

2. School Enrollment Form

This form can be avoided by taking a screenshot of the Demographic Information Screen in PowerSchool on a PC. The mobile app does not support this.

3. Medical Release Form

This form must be completed and  submitted to your coach at the first practice.
