This is for all athletes, volunteers, & coaches signing up in ADVANCE

The Registration (7-8 yrs - $45, 9-10yrs - $60, 11-12 yrs - $60 13-14yrs - $70) fees help cover the cost of fields, umpires, accident & liability insurance, league charter fees, and game balls.

You will NOT pay this fee online. You will have to send it back to the school along with the original form. MARK THE BOX THAT SAYS YOU FILLED OUT REGISTRATION ONLINE and please make sure your players name is on the form.

By filling out this online registration the parent/guardian hereby gives consent and approval for the player(s) to participate in the Benton Little League for participation including transportation and do hereby waive, release, absolve, agree and indemnify to hold harmless the supervisors, participants, coaches, assistants, and individuals both in transportation and activities associated with the SEMO North Little League for any claim of injuries or damages, except to the extent in the amount covered by accident insurance.

I, (Participant’s Name), on behalf of myself, my estate or heirs and any other person
who may have a claim as a result of my death, injury or disability (all hereinafter “RELEASER”), hereby forever release,
discharge and covenant not to sue the Northern Bollinger County Athletics Association and any of his/her/ their/its
directors, officers, employees, insurers, and agents (all hereinafter “RELEASEE”), from all liabilities, claims, demands, or
causes, that I may hereafter have for injuries or damages arising out of my participation in summer ball including but not
limited to losses caused by the passive or active negligence of the release.
“RELEASER” understands and acknowledges that the activities of the Northern Bollinger County Athletics
Association have inherent dangers that cannot be eliminated. Releaser expressly and voluntarily assumes all risk of
personal injury, permanent disability, or death sustained while participating in our summer program including the risk or
active negligence of the “RELEASE” or any other participant.
“RELEASER” acknowledges and fully understands that each participant, including Releaser, will be engaging in
activities that involve risk of serious injury, including permanent disability or death, and that social and economic losses
may result not only from Releaser’s own action, inactions, or negligence, but also from the actions, inactions, or
negligence of the others notwithstanding the rules of play or the condition of the premises and any equipment used.
Risks associated with the ACTIVITIES of the Northern Bollinger County Athletics Association include but are not limited to
2021 further, Releaser acknowledges and fully understands that there may be other risks not known or not reasonably
foreseeable at this time.
Please understand that by completing this online registration, you do hereby represent that you are the parent or legal guardian of the
participant and acting in such capacity agrees to the terms and conditions of the above stated waiver and
release.
This agreement is effective for the following date: January 2026 through December 2026.

DO NOT COMPLETE UNLESS YOU HAVE READ THIS AGREEMENT
