On Field Volunteer ApplicationAfter being accepted as an On Field Volunteer you must complete the following:
1. Have completed the Mandated Reporter Training module no earlier than June 1, 2025. Completion Certificate must be on file with FBGSA.
2. Have completed the Concussion Training module no earlier than June 1, 2025.
3. Have completed a Live Scan questionnaire/fingerprints no earlier than June 1, 2021 and have a copy on file with the FBGSA Document Custodian.

Live Scan Form can be found here:FBGSA Live Scan Request Form Live scan/fingerprinting can be done at the Fort Bragg Police Department Lobby. They are no longer waiving the fingerprinting fee for all FBGSA Volunteers. FBGSA will reimburse fees for coaches that are approved by the board.

Monday 1:30-3:30pm
Tuesday 9:00-11:00am
Wednesday 1:30-3:30pm
Thursday 9:00-11:00am
Prints are done on a first come, first served basis. Please have your Request for Live Scan form filled out ahead of time. You will need to bring photo identification and pay the $30 rolling fee by cash, check or money order (debit and credit cards are not accepted).
