2025 Tournament Playing Rules


Each All Star team is required to have a complete book/binder containing many documents that have to be completed accurately and prior to the Affidavit meeting.
Please provide the below documents to your team manager.

- Original Birth Certificate for each player **If parent does not have original they can go to any courthouse in Texas to obtain a copy.
 
**8YO  & 9YO can provide a copy instead of original 

- Copy of all personnel's Driver's License (Managers and Coaches)
-Copy of certificate of completion of the Diamond Leaders Training and USAB Development Adult Abuse Awareness Training (Required for all personnel!)  Complete the Diamond Leaders Training here! Complete the Adult Abuse Awareness Training here!

 - School Enrollment Form  (date/signature with a current date) OR Proof of residency documents outlined here. 

- Volunteer Application  ***Managers and coaches only. The league can provide this.  

- All Star Player Verification Form ***There can be NO scratch outs or white outs on this form!! League ID # 3431804

For Coaches to use: Player Verification checklist

- All Star Medical Release Form ***Should be dated any date between November 2024 and February 2025 at the bottom

- Parent Code of Conduct Form ***Must be completed for each player!

Your binder should be in the following layout using dividers for each section and page protectors for each form front and back:
