1x1 color picture  (no hats or sunglasses)
2 different proof of residency
current physical on AYF form  (orig & 2 copies)
Report card - end of current school the year   (3 copies)

RETURNING player:
1x1 color picture  (no hats or sunglasses)
current physical on AYF form  (orig & 2 copies)
Report card - end of current school the year   (3 copies)
