Every team must include the following documents (please click on the links below to download a copy):

1) Team Information Sheet

2) Consent and Liability Waiver *one for each player*

3) State/Club Approved Roster

4) Current Player Cards

5) Permission to Travel form - Only needed if your team is outside of Region 1 If outside of Region 1 (CT, DE, ENY, EPA, ME, MD, MA, NH, NJ, WNY, WPA, RI, VT, VA, and WV)

The preferred option to submit these documents is online through your team's GotSoccer account. If you are not able to submit them online, you must come to the tournament HQ 90 minutes prior to start of your first game on Saturday.
