REQUIRED PAPERWORK FOR POP WARNER FOR FOOTBALL AND CHEER

What needs to be uploaded to the 2026 registration portal: 
 
Official birth certificate to be checked & a copy for the book

Recent picture (must show player's face - no sunglasses, etc. hiding face)

2026 Parent/Player Form is now filled out online during registration!

Player’s End of the Year Report Card (Needs to be uploaded and a printed copy to hand in)
 
Pop Warner Medical Form this is the medical form that needs to be filled out by a doctor in order to be eligible!


PLEASE ALSO PRINT A HARD COPY OF EACH DOCUMENT TO BRING TO THE FIRST PRACTICE.
