PA STATE CLEARANCE REQUIREMENTS FOR ALL VOLUNTEERS
Dallastown Cougar Field Hockey is a part of Dallastown Cougar Athletics. Because of this, DCA s required to comply with all state laws applicable to adult volunteers and their interaction with children, including the Pennsylvania Child Protective Services Law (CPSL). In order to be in compliance with these regulations, we must keep on file a full set of clearances for each volunteer.
Even if you have your clearances on file with Dallastown Area School District, or another local youth organization, you will be required to submit them directly to DCA. The same clearances can be used, but must be individually filed with each organization. That said, any clearances submitted to DCA will be valid for all DCA entities - baseball, softball, football, and cheerleading.
Clearances are good for five years from date of completion.
Clearances contain Personal Identifiable Information (PII) and should NOT be shared or submitted to anyone (including coaches or board members) except the DCA Safety Officer. Once all clearances are obtained, please submit electronically all at once to [email protected].
Below is a checklist of the requirements and on the following page is how to obtain all four of the clearances. For your convenience, pages three and four are copies of the FBI Clearance Exemption form (If this applies to you having lived in PA for the last 10 consecutive years) and a copy of the required Mandated Reporter Form.
Each of the following clearance forms (four total) must be completed and are required.
