REGISTRATION INFO

The following documents will need to be turned in before the season begins. An athlete's registration is NOT complete until all documents are turned in.

1. Recent Athlete Color Photo (Head and Shoulders ONLY taken within 1 year; Must be clear with even lighting against a plain white/light background; NO sunglasses, head coverings, additional people, animals, glamour shots, or filters; Athlete must face forward with eyes open and a neutral facial expression; uploaded to the Athlete's Sports Connect profile)

3. 2024 Pop Warner Youth Participant Medical History Form

4. 2024 Athlete's completed Physical Health form (with Doctor's signature and all pages uploaded as 1 document to the Athlete's Sports Connect profile); ANY approved medical/sports physical form can be used.

5. 2024 West Orange Pop Warner Registration Form (with Parent/Legal Guardian signature and uploaded to the Athlete's Sports Connect profile)

6. 2024 West Orange Pop Warner Code of Conduct Form (with both Parent/Legal Guardian signature and Athlete's signature and all pages uploaded as 1 document to the Athlete's Sports Connect profile)

7. Copy of your athlete's Birth Certificate (MUST be a clear copy showing the official seal and uploaded to the Athlete's Sports Connect profile)

8. Copy of your athlete's end-of-year full-year Report Card (MUST be a clear copy showing student's name, student's school, and the grades for all 4 quarters/marking periods and all pages uploaded as 1 document to the Athlete's Sports Connect profile)
