Accident Reporting
- Report the incident to the League Safety Officer and complete the indicated paperwork within 24-48 hours of the incident.
- Document the incident with as much detail as possible using the ASAP Incident/Injury Tracking Report.
- If medical attention is needed, complete the Accident Notification Form and provide it to the family. You can use the Accident Notification Form Instructions for help completing the form.
