The paperwork and documentation required by Pop Warner is extensive, but this ensures that all teams are in compliance with the rules and policies set by Pop Warner National.
The following documents are required by Pop Warner before your child can participate in the season:

-- Final Report Card (2025-2026 School Year)
BIRTH CERTIFICATE
Pop Warner requires that a member of our governing league, FYD Pop Warner, verify all participant birth certificates. The Dunedin Jr. Falcons Compliance Officer will collect your child's original birth certificate, make a photo copy, and take it in person to be verified. FYD staff must have the original or a certified copy (with a raised seal) to look at, photo copies will NOT be accepted. Once they verify the authenticity of the birth certificate, they will stamp the copy which will then be added to the Team Book for the season. After FYD has stamped the copy, the original will be returned to you. Once your athlete's birth certificate is on file, you will not have to provide it again for future seasons.
Valid Passport or Military ID are also acceptable in lieu of a Birth Certificate.
If you have legal guardianship of your athlete, you will need to provide a copy of the court document granting guardianship, this has to go in the Team Book and FYD will require it.
Returning athletes will have their birth certificate on file so this document is not required for registration.
ONLINE REGISTRATION FORMS
During online registration, you will electronically sign two forms - Youth Participant Medical History Form & Participant Contract and Parental Consent Form.
▪️ Make sure the participant's name exactly matches their birth certificate.
▪️ Make sure the name of parent/guardian exactly matches the name listed on the athlete's birth certificate or guardianship documentation.
▪️ Both forms should be signed using a parent/legal guardian's name - do not sign the forms using your athlete's name.
▪️ Emergency contact has to be someone other than the parent or guardian and should not be the same person who signs the forms.
REPORT CARD
Athletes will need to provide a clear and legible copy of their 2025-2026 Final Report Card.
The report card must have the following to be acceptable:
▪️ Participant's Name
▪️ School Name
▪️ Student's Grade Level
▪️ School Year (2025-2026)
▪️ All 4 Academic Quarters or entire year's grades
Athletes entering Kindergarten are exempt from the report card requirement but must complete an Exemption Form.
Students who are home schooled must complete the Home School Form and parents will be required to provide home schooling credentials.
You may bring a copy of your report card to one of the paperwork collection days or email a clear copy of the report card to [email protected].
[ SCHOLASTIC FITNESS REQUIREMENT: In accordance with Pop Warner’s national standards, all athletes must maintain satisfactory academic performance to participate. Report cards will be reviewed prior to certification, and failure to meet scholastic requirements may result in ineligibility until standards are met. ]
PLAYER PHOTO
We will need a recent photo of your athlete to upload into Pop Warner National's roster. We will take a picture at Equipment Distribution, Cheer Uniform Fitting or during practice.
