Novi Bobcats

Every football player and cheerleader must submit a doctor signed sports physical form before the Bobcat season begins. This physical must be dated after April 15 of the current year, and must indicate that your participant is in good physical condition.  (Following MHSAA guidelines is preferred, but we will accept any physical from the current calendar year.)  A link to a printable sports physical and medical history form can be found HERE and you can also find one under the Resources tab.


We are paperless!  ALL PHYSICALS & MEDICAL HISTORY forms must be scanned and emailed to [email protected].



URGENT CARE offers SPORTS PHYSICALS
If you are unable to secure an appointment with your family doctor or pediatrician before the start of the season, this may be a good option for you! Be sure to print and bring along the sports physical form when you go - Urgent Care facilities do not always have sports physical forms on hand.  Fees range $25 - $30 depending on the clinic.

1. PrimeCare Urgent Care of Novi - 39555 W. 10 Mile Rd, #301. 248-426-9680. M-F, 8am-6pm. Sat 9am-2pm. Sun Closed. (Call ahead for an appointment - or walk-ins are welcome.)

2. IEP Urgent Care of Novi - 27204 Beck Rd. 248-513-3719. Make an appointment online at https://www.iepurgentcare.com/novi .  Appointments have priority over walk-ins.  M-F, 8am-8pm. Sat & Sun, 8am-6pm.
