Complete online registration (ATHLETE IS NOT SIGNED UP IF ORDER IS ONLY IN THE CART)
Copy of birth certificate
Final report card
*****DOCUMENTS MUST BE SCANNED COPY, PDF OR PAPER COPY. CELL PHONE PHOTOS NOT ACCEPTABLE. USE SCANNER APP ON PHONE IF NEEDED.
