Form Turn In events:

After you pre-register your player, we need a copy of the Player Registration Form. The form is what we use to build teams and to provide coaches important information. If you did not print the form already, log in to your account on www.eayso.org, select the player, go to FORMS. You will get a pre-filled PDF.

IMPORTANT: Spring is a continuation of the Fall outdoor program so new players will be joining existing teams.  We will make assignments as space is available.  If you have other afterschool commitments that could interfere with an established team's practice schedule, write that on the form. Meet our registration volunteers and turn in forms/outstanding payment at these times:
