San Diego Bolts would like to say thank you to all who have volunteer to mentor these young men and women

ALL COACHES and/or STAFF ARE REQUIRED TO USE THE ONLINE REGISTRATION
CERTIFICATIONS ARE REQUIRED TO COMPLEATE ONLINE REGISTRATION

PROFILE PHOTO REQUIREMENTS

1. A passport style photo (head Shot)
2. No hats or sunglasses and (he/she) must have a shirt on.
3. Tip set your phone camera on square or 1:1.

Football Team Staff

(1) Head Coach (age 21+), (4) Assistant Coaches (age 18+)
(1) Coach-Trainee (student, age 16-17)
(1) Equipment Manager, (1) Trainer,
(1) Business manager, (1) Team Parent (age 18+)

Will need to complete the following

Live Scan WCCPW Form
Mandated Reporter Training (Sports & Recreation Volunteers)
Heartsaver Pediatric First Aid CPR AED In Person Class prior to the start of practice.
(No ONLINE Class. CPR Card must have a QR Code on it or it will not be accepted)
USA Football USA Football Club Code (SDUAA)

Cheer Team Staff

(1) Head Coach (age 21+)
(4) Assistant Coaches (age 18+)
(4) total, Coach-Trainee (student, age 16-17) and/or Student Demonstrators (student, age 15-17)
