BEFORE YOU REGISTER.....

Medical release form - Please download and complete the medical release form. We must have an updated form on file for each player, every year.

THPRD numbers - We are required to provide THPRD with a Member ID number for each player. Please have your number handy when registering. (If you do not yet have a THPRD number, you can find information here.)

Payment assistance - If you have, or plan to, apply for financial aid through THPRD, please contact us to coordinate. Additional information about THPRD's financial assistance program can be found here.
