Code of Conduct

  • Will be asked to leave the facility and be suspended for the following game and practice
  • Repeat violations may cause a multiple game suspension or the season forfeiture of the privilege of attending all games and practices.

Concussion Fact Sheet

Injury Report must be completed by a coach within 48 after a game for all injuries needing medical attention. An injury is when any player, manager, coach, umpire, or volunteer has to receive medical treatment and/or first aid, even for passive treatments (such as the evaluation and diagnosis of the extent of the injury or periods of rest). Examples of injuries:

  • Unconscious player
  • Cuts, Scrapes and Bruises (minor or profuse)
  • Cut lip, gums, or tongue.
  • Strains and Sprains
  • Fractures
  • Insect Bites and Stings
  • Knocked out or Fractured Teeth.
Volunteer Forms must be completed every year.  There is NO CHARGE for this.
