REGISTRATION PROCESS & REQUIRED DOCUMENTS

The documents listed below are the REQUIRED by Little League International for all players (the underlined documents below are hyperlinked).

Birth Certificate - District-5 is required to review the original birth certificate, then certify a copy for our records. Once we obtain a District-5 "certified copy" of your player's birth certificate, this requirement will be considered fulfilled for the duration of your player's Little League career.

Medical Release - The medical release form must be completed each Spring, for every player. This document will also be valid for the Fall season of that same year, if you register your player for Fall Ball.

Photo / Media Release - CHLL may post photos of players on our website and social media pages. A waiver must be signed annually each spring to release CHLL from any liability. If you prefer not to have your player's photo posted, please reach out to our Player Agent(s).

Proof of Residence Requirements - 3 documents are required to fulfill this requirement. Please review the attached instructions carefully. The necessary documents are date-sensitive, and District-5 will reject them if they do not meet the exact requirements.  **Because this requirement is so complex. We highly recommend that families use the "School Enrollment Form" instead, when applicable**
